The 38 would need a rebore to 375 as a cap & ball barrel won't fit the conversions or Opentops.  Everything will align, as the cylinder is already timed to the gun to start with, as ling as the rebore stays centered.

In concert with your other thread, the 44 Russian, Special or Colt cylinder, reamed out and mated to a 45 Colt or 45 Schofield barrel would do to convert a 44 Opentop or R-M.  If the Opentop is already a 45, just get a 44 cylinder and ream it for heel based, as you suggested.

As Flint said;

    Rebore to approx .375
    (I believe that someone who could rebore for 38-55 could do the job as long as the twist was correct)

    Or turn a scrap .36 cap and ball barrel down to reline the original barrel.

The cylinder is pretty simple, just use an inexpensive chucking reamer of about .380
