Magazine explosions... A modern problem at all?

So I know some magazine explosions occurred historically, but are they an issue at all with a blunt follower and flat nosed bullets?  An arm-chair firearms "expert" I have an acquaintance with was regaling me with stories of people "gettin' thar heads blowed off" (yeah, that's a quote) recently using Spencer's.  I have never heard of such an incident.  Anyone know of any, and know the circumstances? I mean, I could see it if someone were stupid enough to load a pointed bullet, or used the original magazine follower....

This is the closest thing I can find.  I wonder if it was an origional follower being used.

That seems to confirm my suspicious that the problems seem to be related to bad ammo (high primers) or using an unaltered original primers, or pointy bullets.

I value my head enough to inspect ammo and use the right gear!
