What does this tool do?

Mornin' Duke,

The crease is a line burnished in the leather along an edge.  The shoulder (if you will)  guides the tool along the edge of the leather while the sharp edge burnishes a line in the leather.  This tool has several guides to place the line anywhere from 1/16" to 1/4" from the edge.   Make sense?.....
